Shoulder Pain at Night: Causes and How to Sleep With It

Shoulder pain is often WORSE at night because lying down puts pressure on the sore spot, lets the arm sag into positions that pull on it, and takes away the distractions that dull pain during the day.

The usual causes are a sore rotator cuff, frozen shoulder, calcium in a tendon or shoulder arthritis. All four are common, and most get better with the right exercise and time. Setting up your pillows well can buy you better sleep while that happens.

Night pain on its own is rarely a sign of something serious. It matters when it comes with chest pain or breathlessness, or when nothing you do eases it.

Why shoulders hurt more at night

  • Pressure. Lying on the sore side squashes the tendon and the fluid sac above it between your body weight and the mattress
  • Arm position. Lying on the good side lets the sore arm drop forward across your chest. Lying on your back lets it sink backward. Both can pull on sore tissue
  • Stillness. Hours without moving let a stiff shoulder stiffen more, so turning over hurts
  • Fewer distractions. In a quiet, dark room your brain pays more attention to pain signals

Which one is it? A quick guide

  • Rotator cuff problem: ache on the outside of the upper arm, painful lifting the arm out to the side or overhead, worst lying on that side. Someone else can usually lift your arm all the way. See rotator cuff tendinopathy
  • Rotator cuff tear: the same pattern plus real weakness lifting or turning the arm. Pain at rest and at night is typical, especially lying on that shoulder
  • Frozen shoulder: a deep ache and a shoulder that gets STIFFER over weeks, until nobody can move it fully, not even your physio. Turning the arm outward is usually the most restricted. Night pain is often worst in the early stage. It’s most common between 40 and 60, in women, and in people with diabetes. See frozen shoulder
  • Calcific tendinitis: calcium deposits in a cuff tendon. The pain can go from mild to severe over a day or two, keeping you awake even when you’re lying still. See calcific tendinitis
  • Shoulder arthritis: worn joint surfaces, usually in older adults. Stiffness in all directions, grinding or clicking, and a deep ache that disturbs sleep as it gets worse. See shoulder arthritis

These overlap, and more than one can be present. A physio or doctor can tell them apart by testing how far your shoulder moves, both on its own and when someone else moves it, and how strong it is.

How to sleep with shoulder pain

  • On your back: put a pillow under the whole sore arm so it rests level with your body or slightly in front of it, not sinking back toward the mattress
  • On your good side: hug a pillow to your chest and rest the sore arm on it. This stops the arm dropping forward across your body. A pillow behind your back stops you rolling onto the sore side
  • Propped up: sleeping half-upright, with a wedge or a stack of pillows or in a recliner, takes pressure off and helps many people through a bad spell
  • Avoid the sore side for now, and try not to sleep with the arm overhead or tucked under your pillow
  • A body pillow can do several of these jobs at once and makes it harder to roll onto the bad side

Pillows get you through the night. Exercise is what makes the nights better.

What else helps you sleep

  • Time your painkillers. If you use them, taking them an hour or so before bed covers the first hours of sleep. Check with your pharmacist which are safe for you
  • Heat or ice for 10 to 15 minutes before bed, whichever feels better
  • Gentle movement in the evening. A few easy pendulum swings, letting the arm hang and circle loosely, can ease stiffness before you lie down
  • Don’t overload it late in the day. Heavy lifting or overhead jobs in the evening often mean a worse night
  • Keep up your daytime exercises. Strengthening for cuff problems and stretching for frozen shoulder treat the cause
  • Ask about an injection if night pain is stopping you sleeping for weeks. A steroid injection can calm pain for a while, which can make sleep and exercise easier

  • The shoulder is hot, red or swollen and you feel unwell or have a fever. Joint infection is rare but serious
  • You fell or hurt it and now can’t lift the arm, or the shoulder looks out of shape
  • Pain has suddenly become severe over a day or two

Get it checked if

  • The pain is constant and nothing eases it, whatever position you’re in, day or night
  • You’ve lost weight without trying, feel generally unwell, or have had cancer before
  • You smoke or used to, and have pain around the shoulder blade, especially with a cough that won’t go away. A tumor at the top of the lung can cause shoulder pain
  • There’s a lump around the shoulder or upper arm
  • The arm feels weak, not just painful
  • There’s no improvement after 4 to 6 weeks of exercise and sleep changes

Night pain tells you the shoulder is irritated. The pillows buy you sleep, and the daytime work gets you better.
